High sugar at night and in the morning - advice please
Hi, this is my first post. Thanks to diabetesforum/com for providing free forums for asking questions.
Here is a general log of my sugar for the past few days. I just switched to insulin (Humalog and Lantus) and am not getting satisfactory results especially during the evening and in the morning. I was prescribed 4 units of humalog at meals and 10 units of lantus at bedtime (10 PM eastern).
Can anyone figure out what I'm doing wrong? I've even tried doubling to 8 units of humalog recently but didn't see any difference in the sugar level. I am injecting with 8mm needles into my stomach, I heard injecting into your arm gives more bang from the insulin.
Any advice would be nice. Thanks!

Hello, welcome to our community! Do you use "carb counting" and determine your Humalog dosages by knowing how many carbs you are going to eat? I had many highs before using carb counting but after determining my dosages in this way my control improved very much. I started having A1c's below 6.0 due to carb counting. If you don't use carb counting then a diabetes educator and a dietitian can help you with it and get you started. I use a 1:6 ratio for most of the day. That means I take 1 unit of insulin for every 6 carbs I eat. If I eat a 42 carb meal then I need 7 units of insulin. I also use insulin before snacks in the same way. In your case you would need a different ratio for evenings to give you lower blood sugar at that time of day.
It sounds like you are also having trouble with "Dawn Phenomenon". Here is a link that might help you with that high fasting sugar level in the morning.
Ok so I had a chat with my diabetes educator and she said that I'm supposed to monitor sugar before meals and at bedtime. Also I'm supposed to inject before eating not after.
I'll try this out for a few days and post an update.
Thanks for the response :)
How do you take your injections as you should take your bolus before you eat, Not after. ;) With your basel, do you just take the one shot in one day?
Latest work on diabetes
I need some latest information on diabetes I am diabetes typ 2 and in very good control i using amryl 1mg from last 5years in winter i use 0.5 in morning and 0.5 in evening and summer i just use0.5 only in morning some time not every day.
Thanks for your help. I inject now 4 times in a day, 4 units / humalog at meals and 12 units lantus at 10PM.
Since I started injecting before meals my sugar has become much closer to target levels, some slight adjustments might be needed.
Yesterday I had a poutine with 10 units humalog (potatoes and gravy) and my sugar was 7.2 just before my next meal (which is quite acceptable for me). At bedtime it was 9.4 and this morning before breakfast was 8.3.
I inject into my stomach, and I've heard that injecting to a part of the body with less fat has a stronger effect, is this true?
fehmida> If your sugar is in good control, what information would you possibly need? Please provide some details as to what you're looking for.
Also, does anyone have any idea how much carbs are in white rice, and should I drastically increase my humalog shot to compensate (like for the poutine yesterday) when I have it?
I suggest that you buy a copy of the "Calorie King". It gives the carb content for almost everything you eat. I bought mine from amazon.com.
I'm beyond frustrated now and am ready to throw away all these needles because I feel the shots are not doing anything.
Lately I have been bumping up my shots of humalog (now to about 20+) and lantus is now at 24 units.
My morning is always high around 9-10 (probably around 160-180 for the US), lunch around 7-8 (130-150) and supper/dinner is usually 9-10 (same around breakfast).
I'm close to around 290 or possibly 300 pounds, and I am 6 feet tall. I get almost no exercise because I sit in front of a computer all day (helpdesk/NOC) which takes up my entire day and when I'm home I have to take care of my son who's about a year old now.
Am I doing something wrong, or am I ultra insulin resistant?
