The answer is.... it depends on your own reactions.

I can sometimes "get away with" some deviation, but then, sometimes I can't. My reaction to gluten lasts about 3 days. So I usually know when I've gotten "glutened". The longer I've been gluten-free, the less severe my reaction is. However, this is an individual thing, and some people have a more severe reaction.

Re: coming forward? Most doctors don't recognize gluten intolerance because they can't test for it. Even the "gold standard" biopsy of the small intestines can miss damage and so come out negative. In my own case, since I had GI for so many decades undiagnosed, and did not seem to suffer from any damage, I suspect my version with my genes has not caused any physical damage (other than malnutrition because of losing most everything I ate).

I had no idea you couldn't test accurately for gluten intolerance! Learn something new every day.

I find that if I eat bread or pasta, my stomach HURTS. Last time it felt like my stomach was actually throbbing. Yet if I eat ice cream, my BG will go up but I don't get other symptoms. Probably due to the higher fat content too.

Ice cream probably doesn't have any gluten in it.

Stomach hurting after eating gluten foods (pasta, bread, etc.) is a SURE SIGN that one is intolerant to gluten.

There are many symptoms of the autoimmune response to gluten. While GI issues (diarrhea, bloating and cramps) are very common, not everyone experiences this. Your symptoms are quite common, also.

Here's another thing about untreated celiac or gluten intolerance.

When the little villi of the small intestines are damaged from the gluten, the damage starts at the tips, which is where lactase is made. Many undiagnosed Celiacs are also lactose intolerant because of this, but when they go gluten-free, the villi heal and they are once again able to digest lactose.
