Well, here's what I know:

- if you equalize the amounts, big tomatoes typically have less sugar in them than grape tomatoes (they're called "grape" for a couple of reasons...). But in the portions you're using them, it may be a wash.

- not all dressings are full of sugar, but many can contain more sweeteners than you might think and a "serving" is considered to be just 2 tablespoons. So it may be that you're using double or more dressing on a salad that size. That could be another 8-12 grams of carbs over what you put on a sandwich.

- the Hershey bar is a bit of a conundrum to me; I could not eat a Hershey bar and not notice. Heck, I can't even have more than a square or two of 70%-85% dark chocolate and not notice it (not a spike but a bump). But that's what makes all of us different and more power to ya!

My SWAG here is that by the time you add up differences like the carb count of the lettuce used in a bowl instead of on bread, the amount of dressing used, etc., the differences may be just a few carbs here and there but, together, they add up to a substantially different number.
