I just use an Excel spreadsheet I made up for myself. I print out a two sided copy and carry it in my meter case. In very condensed form I keep track of my tests, carbs, amount of sleep, weight, and everything I eat - also noting any weird feelings or illness. That one piece of paper holds everything for about 16 days. Then I enter just each day's fasting and high reading into another spreadsheet.
I also made myself a tiny chart of the foods I'm likely to eat and their carb/fiber counts and another paper lists all the supplements and medications I take and my latest lab test results. These all fit in the pocket of my meter case.
